Registering for the 24-25 school year?

It is recommended that you use a computer to complete this process, not a mobile device. This will allow you to more easily access and view documentation. If you have questions or any challenges, please contact the main office at your child’s school and ask for the attendance secretary. If you are unable to reach someone at the school, please call the Student Support Services Department at the central office, 615-893-2313, for additional assistance.
Only a custodial parent or legal guardian may register the child. The custodial parent or legal guardian must show proof of guardianship by presenting a copy of custody papers or a court order.

Returning Students (Grades 1-6)
Tuesday, April 9th

What documentation is required to enroll my child? Any student entering the MCS school system for the first time must present the below items to their school. All forms must be submitted prior to the first day of school.
- A birth certificate or officially acceptable evidence of date of birth at the time of registration.
- Tennessee Dept. of Health Certification of Immunization
- Proof of Physical within the last 12 months. A Tennessee Department of Health “Official Immunization Certificate” must be provided to the school before the student’s first day of class. The certificate is available from the local Health Department or your child’s physician. Students who are exempt from immunizations are still required to have this certificate signed by a physician and have proof of a physical exam. This includes students entering Pre-K, kindergarten and students transferring from another state.
- Proof of Residency: Each year, parents are required to show one form for proof of residency from the prior month, with name and current address on the document. These are the eight acceptable items:
- Water
- Garbage
- Electric
- Mortgage statement
- Rent/lease agreement if less than 30 days from origin and confirmation of utility services.
- Close of escrow “Final Buyers Statement” if less than 30 days from move in date and confirmation of utility services.
- If residing with another family, the primary homeowner must fill out a notarized Residency Affidavit [visit this link, Affidavit Forms, to get the files in English, Spanish and Arabic. There are two files (overview and form) in each language]; provide two proofs of residency under his/her name, and a photo I.D; however, proof of residence or guardianship may be requested at any time by the school administrator. Please contact the main office of your child’s school if you need this form.
- Parent/guardian’s state or federal issued identification.
- If parents are divorced, separated or unmarried, current proof of custody of the child.
- Special Notice: Tennessee Law prohibits the fraudulent enrollment of students, including providing a false address or fraudulent affidavit. Any parent/guardian who fraudulently enrolls a student shall be liable for restitution to the school district for an amount equal to the per pupil expenditure for the district in which the student is fraudulently enrolled.
